DSA Open Enrollment


Open enrollment materials for DSA benefits were mailed to your homes the week of October 15th. All members must re-enroll in dental and vision plans. The current DSA plans will end on January 1, 2008.

Active members:All Enrollment Forms need to be completed and returned to the DSA by November 30th.

Retired Members: All Enrollment Forms need to be completed and returned to the DSA by November 30th. Benefit changes made during this Open Enrollment period are effective January 1, 2008 and will be in effect through the end of the year.



Department


County Open Enrollment begins October 15, 2007 and ends November 15, 2007. Benefits selected during the October Open Enrollment will take effect for the January 1 through December 31, 2008 plan year.

This year’s open enrollment information will be mailed directly to your home by the county.

This is your chance to change your benefits through the County. The DSA also offers Vision and Dental Insurance.

For DSA Benefits contact Twyla Curl at the DSA office for more information or to enroll in Dental or Vision Insurance.

For County Benefits:
STEP 1: Carefully read the Open Enrollment materials mailed to your home to make your benefits selections.

STEP 2: Complete your Personal Worksheet. Circle your benefit selections for the new year. Have your Worksheet available for Step 3.

STEP 3:
Call the Open Enrollment Telephone Line 877-530-0016 if either of the following applies:

* You want to contribute Out of Pocket to a Flexible Spending Account
* To make any other benefits changes

Key in your selections on a touch-tone phone. Make sure to stay on the line until you receive a confirmation of your elections. The Open Enrollment telephone line will only be available between July 23 and August 13.

STEP 4: A Confirmation Statement will be sent to your County Mail Stop. Please review it to make sure your elections are accurately reflected. Sign and return it to MS A-7.

STEP 5: Submit an Enrollment Change Form (if requested on your Confirmation Statement) to Employee Benefits to complete your new plan year elections.

To make changes to Domestic Partner Benefits, do not use the Open Enrollment Telephone Line. Instead, please contact Employee Benefits at 888-550-2203.

  • Remember to use your PeopleSoft Employee ID number and the last four digits of your social security number to access the phone lines. Your Employee ID will be printed on the Personal Worksheet you receive with your Open Enrollment materials.
